Boss Lair Rainbows
Most bosses drop from the same shared mythic pile. The named pieces people call Sissirat's or Manglefeet's are not on that pile. They sit on the chest in that boss lair. The Wishing Well can spit the same names once you are high enough for that dungeon.
Drop odds for the shared pile, the ordinary chest roll, and this special chest roll are on Rainbow Drops. Which shared-pool names can show at your level is on Rainbow Level Bands. Names that only come from the Well are on Well-Only Rainbows.

How it works
From Algernon on, the treasure room after the fight has a boss chest. That chest still rolls ordinary loot, including a slow shot at the shared mythic pile. On top of that it has a special about 1 in 180 for one name from that lair's short list.
Rattle Tooth in Dew Valley has no boss chest and no named list.
These named pieces are not in the shared table every boss corpse rolls. Killing Sissirat will not make Manglefeet's chair show up on the body. You want that chest, or the Well once you are in that dungeon's level range.

Level floors
The named chest roll will not fire below the character level in this table. There is no maximum. A 100 still gets the same list. The piece comes out at your level.
The Wishing Well uses the same dungeon floors. Early lairs still sit on the mythic floor of 15 even if you can walk into the dungeon sooner.

Five on Insane is about 1 in 14 for some name from that list, and about 1 in 43 for one specific name when the list has three. How loot percent, party split, and under-level groups change that is on Boss-Exclusive Rainbow Farming.
HDoL
Bosses in the Horrific Dungeon of Legend still roll the shared pile, just hotter. They do not have their own unique rainbow names. Odds are on Rainbow Drops.
