These names do not come off a boss corpse, and they are not sitting on a lair chest. You get them from the Wishing Well. Insane and loot percent do not help. The Well does not care what dungeon you just left.

The Well has an extra roll on top of the ordinary yellow / purple / rainbow junk, about 1 in 20 per item. A paid toss is two items, so about 1 in 10 tosses you will see that extra bag at least once. The bag is a mess: pirate gear, the mouse ears, Charge!, NC Hammer, a bunch of named boss pieces, books, a gold brick. Armor comes out of it more than weapons do. So 1 in 20 is "something from that bag," not Squeaks.

Named lair pieces and Nacho Supreme can also come from the Well once you are high enough. Those are on Boss Lair Rainbows. They are not only-Well. Nacho is the odd one — the Well can spit it from 15, Manglefeet's chest waits until 80.

The long math, including why a low-level toss can make a specific name hotter (shorter bag) without changing the 1-in-20, is on Wishing Well Guide. Ordinary boss odds are on Rainbow Drops. Names that no longer drop anywhere are on Discontinued Rainbows. Holiday pack names and the snowman hats are on Holiday Rainbows.

Pirate set

The pirate look is Well-only. You will not see these names on a boss corpse or a lair chest.

Puffy Shirt of the High Seas
Sea Bird Projectile Protector of the Scallywag
5 Fleshy and 1 Metal Hook Holders
Salty Toes of the Low Tide
Portable Poop Decks

Helms

Squeaks and Doesn't Squeak are the mouse-ear pair. They still drop. From 15.

Weapon

NC Hammer

Not rainbows

The ninja look from the Well is unique (purple), not rainbow. Same Well, different color.

Seasonal Well

When Karl's 1st Limited Edition Item Pack look is on, the Well grows a separate holiday table. That is not this list. Nineteen 1st Edition rainbows, plus the Daily / LARP purples. The snowman hats are a different pile — they sit in the everyday extra bag and come off the winter quest. Both are on Holiday Rainbows.

Rough odds

Worked out on Wishing Well Guide. Short version, per paid toss:

Pirate armor or Charge!: maybe 1 in 1,500 to 3,500.
Squeaks / Doesn't Squeak: maybe 1 in 3,000 to 8,000. The purple hats hog that helm list.
NC Hammer: maybe 1 in 2,000 to 6,000. A bit hotter before 80, before the chair sits down on that mace list.

The 1 in 20 is the extra bag. The name is the long part.
